Measure his left ankle and multiply it by the square root of pi. Make sure it's in inches though.
I'd have said get a tape measure and measure the inside collar of one in the wardrobe making sure the buttons done up, but my first idea is easier.

It works as any slim fit XXL shirt would, if you take XXL then you are likely a 48-50 inch chest and would do better wearing something looser fitting unless you like the moob look.
