On 8th October you'll fly from Birmingham to Bangkok with KLM, arriving on the 10th October as these flights have a full day and night in Amsterdam which is a bonus in my eyes! You'll overnight in Amsterdam at the Park Inn which is at the airport but it's easy to get into the centre. You'll then fly on 9th October to Bangkok, arriving on 10th where you'll spend just two nights in Bangkok at the River View Guest House which is highly rated and near the riverbank and Chinatown area of Bangkok ideal for getting around but in a quiet area.
On the evening of 12th April you'll take the sleeper train to Chiang Mai. You'll arrive in Chiang Mai on the 13th October and spend 4 nights in Chiang Mai at the highly rated Romyen Garden Resort. There should be plenty of time to enjoy an elephant trek or even head up to the hippie town of Pai, do some hiking or hire a moped.
On 17th you will fly from Chiang Mai to Phuket with Air Asia. You'll spend just 1 night in Phuket at the highly rated Little Moon Residence as you'll be catching the ferry on 18th, the next day to Phi Phi. On 18th you'll catch the ferry to Phi Phi and spend 5 nights chilling out at the Viking Nature's Resort which again comes highly rated. I've based you there for 5 nights but if you wanted you could also head to Krabi and split your time but I think Phi Phi is nicer in terms of accommodation available.
On 23rd you'll catch the ferry back to Phuket where you will stay for another night at the Little Moon. If you left your main luggage there you can also collect it. On 24th you will fly back to Bangkok for a final night at the River View Guest House before your flight back home.
This is just as example and you could adjust this to suit. There are cheaper options in terms of accommodation but I've picked ones that are highly rated. You could also take bus journeys which may be cheaper, although longer than flying.
