I've had it for the past year and I haven't had to buy anything.
The only upgrade is the Patient Education package which is £18.49. That is only designed for medical professionals to educate lay people on medical conditions etc.
There are other apps such as Anatomy and Physiology and Muscle Premium which are more useful to medical students etc.

There's Human Anatomy Atlas 2017 for 89p (not showing as discounted), and Human Anatomy Atlas for £20.99. Does anyone know the difference between the two apps?

Looks like a similar deal is available on iOS for 99p, although the iOS edition appears to have been sent back from thefuture.
