My Sandisk Extreme card also failed after about 18 months of use. Locked into read only mode and not possible to write to it anymore. Just a waste of money.

Formatting doesn't work. Apparently when these Sandisk cards develop a fault with the memory child, they automatically lock into read only mode. Nothing else is possible. I'm buying Samsung, Lexar or something else next time.
