Yes it does. There is however, some faff on getting it to work on some machines. Go to the Steam discussions section and the top stickied post tells you all you need to know to get it working.
It also goes into how to mod it too, which seems essential going by most people's thoughts.

It is not optimised for Windows 7 and later because it was released about a year before Windows 7, but that doesn't mean it doesn't work - and if it doesn't work, it's not necessarily a Windows problem, it could be down to your hardware configuration. For some people it just works, but for those who experience problems there are easy to apply fixes for all the common issues.

I found it played fine without mods. I think I actually got mine to boot on Windows 10 without any of the edits (just win xp compatibility mode) unlike my windows 7/8.1 installs previously. Crashed a lot but that pretty much is fallout 3. The edits help a lot though and take 10 minutes top.

i love this game but i lost my patiance with it crash on a crash in win7 now in win10 ,even win10 is crashing on clean install i thing it got something to do with my system configuration

Now... modding is slippery slope... He'll begin with realistic physics and weather, turn those plastic mannequin heads into people and die from lack of sleep after installing Basic Needs...
(to be honest I can't imagine playing Fallout without couple of mods... never liked F3 (prefer F:NV and - gasp! - F4), but maybe it's time)
