I believe it's per-delivery. The only problem is if you need lots of small things that count as add-on items then you're out of luck.

In my experience it is done at point of dispatch. So if x2 no-rush items dispatch in same shipment, you only get one credit. To get the double credit you either have to wait for the first to dispatch and then no-rush the second item or hope that they are shipping from different warehouses.
One trick (if you want the item quickly) is no-rush the first order and then prime the 2nd - providing they are coming from the same warehouse (which they normally do), you will get the credit but both will ship the in the same next day shipment.

Credits can't be stacked. None of the Ts&Cs say they can't (below) but they still won't apply more than one at checkout.
My first pantry order was due today, I ordered Wednesday night. Just had email saying that it's been delayed until tomorrow. I think pantry still needs a bit of work. They could also make it easier to amend (cancel items and add new ones). Supermarkets still do this better.
