You send it to the USA. They have some stipulations on their website about marketplace sellers needing to have a UK return address when selling items that list international shipping, but Amazon US don't follow that rule themselves.
Also if you are returning it for a refund, you will have to chase customs for the import fee deposit refund, as Amazon US don't refund this themselves.

That depends, the OP has put including tax, it's possible this is worldwide deliver eligible, meaning the import tax is included in the price, it should say on the listing, or you can click on the left hand side amazon worldwide eligible, or something like that....
If the tax isn't included then yes don't buy it!

The price upon shipping includes import tax. I've ordered from Amazon.com before (above the exemption threshold) and you do not get hit with any additional charges.

A waitlist is basically where people 'claim' the deal (ie it goes into their basket and gives them 15 minutes to check out) but then they don't complete check out so their item is released to people on this list in the order that they joined it
