This is the same form from when they gave it away years ago and haven't closed it. No samples will be given out as highly likely all went years ago. If you scroll to bottom of page the copy right is still registered in 2012.

I did this about 5 or 6 months ago still havnt got my first sample went to do it again and it says you have already entered for this , so its pointless
