bear in mind ordinary debit/credit cards have a foreign exchange fee usually 2 or 3 %
I just checked on Revolut and 390zar is currently around £22.75

I believe that £23.10 is already included charges and tax, since 390 ZAR is approx £22.75 as you said.
Should I edit my main post by using the exact price (not including those charges)?
How can i purchase this? Make a new account on switch?

You could change your account region by log in into this website https://accounts.nintendo.com .
If you have any remaining fund in your account on the current store, then you have to spend them all, otherwise they wouldn't allow you to change the region.
Creating a new account is also an option. Many people have separate accounts for US and JP regions. You could look in YouTube, it's quite easy to create a new one.
But for me, I prefer keeping my every purchased history in a single account.

You can change region as long as there's no credit on the account.
So you can use your existing account or you can create a seperate account, as you're able to play purchased games with any profile.

I changed my region but when I try and buy through za store I get an error saying wrong region.

When I try to change to ZA I get the following message:
"Your Nintendo Account and Nintendo Network ID are linked. Changing the country of residence will disable this link."

Your NNID will be unlinked from your Nintendo Account. Can be relinked when the region's match again. (NNID is fixed to region set when created)
Guys you can use the eshop on the console itself once you've changed your Nintendo account region at accounts.nintendo.com

Try a different web browser.
I signed in and changed region via safari on iPhone.
Then swapped over to chrome and it worked

The Nintendo account using for purchase in the eshop is different from the NNID one though. But I think you meant the first one.
