Mine opens upwards and I just fitted these ladders. You just need enough room to push the loft hatch up the opposite end to the ladders. It does come with a conversion kit including hinges to adapt your existing loft hatch, but it assumes you have a wooden door you can adapt. I'll do mine one day...
The hand rail is bolted on
Decent quality ladders, I'm pleased with them.

Am I correct in thinking this will only work on a loft hatch that opens downwards as in the photo? Mine opens upwards into the loft

You could always drill out the rivets if it was riveted on.

Depends on your DIY skills but the 1st part is simply hanging it off the edge of the loft boards with the supplied hinges, then you have to attach the spring arm but I think you get a template. It comes with stops to set the height.
